Having collaborated with Neeltje Vermunt on articles that arose from her recently defended PhD thesis, we have since brought two ideas together into one new integrated model which we expect to become a vital next step in achieving person-centered decision making in complex cases. The article proposing the new model has been sent to a peer-review journal for publication.
The two ideas were the three-talk model of the shared decision was updated in 2017 and the three-level goal model, published in 2018. Shortly, we hope to publish a merger of the two models, and suggest that a goal-based approach to SDM is overdue, especially for patients with complex and multiple long-term conditions.
In creating the integrated model, we found it helpful to prioritize the goals using high, medium and low priority rankings. We then mapped possible interventions onto the relevant goals, using color coding. We developed this into a Goal Board. We plan to develop this into an interactive process over the next year and welcome interest from clinicians who want to try it out with their patients. Comments on the concept are welcomed.
